There is a particular kind of appointment that stops us in our tracks.
A parent walks into our Carmel Valley office with a nervous five-year-old, gets them settled into the chair, and then says — a little sheepishly — "I used to come here. You did my sealants."
It happens more than you would think now. And it is the clearest measure we have of how long we have been part of this neighborhood.
We got here before the neighborhood did
When we opened our doors more than thirty years ago, Carmel Valley was still becoming Carmel Valley. Carmel Del Mar School had just opened as the first campus east of the 5. Torrey Hills was hillsides and grading plans. Pacific Highlands Ranch did not exist yet in any form a resident would recognize.
We set up shop right at the hinge of everything — minutes from Del Mar Heights Road, easy off the 5 and the 56, close enough to Del Mar and Sorrento Valley that a parent could make a 3:30 appointment without rearranging their entire workday. Three decades later, that has not changed. This is still the kind of place where you run into three families you know at the Torrey Hills Center on a Saturday morning — and still the easiest place in 92130 to get to when a kid chips a tooth on a scooter at 4:45 on a Thursday.
We know your school's bell schedule
One of the odd side effects of practicing in one neighborhood for thirty years: you end up with an informal map of every elementary school in the area, built entirely out of appointment requests.
We know that Torrey Hills School families can walk over after dismissal. We know Ashley Falls, Carmel Del Mar, Sage Canyon, Sycamore Ridge, and Ocean Air run on Del Mar Union's calendar, and that a district-wide minimum day means our afternoon schedule is going to be busy. We know Pacific Sky brought a wave of new Pacific Highlands Ranch families through our door when it opened. And we know the Solana Beach side of the neighborhood just as well — Carmel Creek kindergarteners who become Solana Pacific fourth-graders, plus Solana Highlands and Solana Ranch.
We know which campuses do class photos in the fall, because that is when we get the calls about "can we do something about the front tooth before Friday." We know that Carmel Valley Middle and Pacific Trails Middle mark the age when kids start asking their own questions instead of letting a parent answer for them. And we know that when a patient starts talking about Torrey Pines High School or Canyon Crest Academy, we are about two years from a very different conversation about their care.
We are not affiliated with any of these schools. We just love them, because they are where our patients spend their days.
Our doctors live here too
This is not an outpost we commute to. Dr. Kim Lugtu, our Clinical Director and a board-certified pediatric dentist, and Dr. Anna Grindberg, our general dentist, both live right here in the neighborhood — and their own kids are in the same school system as our patients. The families in our chairs are not an abstraction to them. They are classmates, teammates, and the people they see at drop-off. When we say friends and neighbors, we mean it literally.
The second generation
Here is the part we did not plan for.
The kids who were in our chairs in the mid-nineties — the ones who came in from those first Carmel Valley elementary classes — grew up. They went to Torrey Pines. They went to college. A lot of them left.
And a striking number of them came back.
They came back to raise families in the same neighborhood they grew up in, and they brought their kids to us. Some of them remember the fish tank. Some remember which prize they picked from the treasure box. Some remember being genuinely scared and being talked through it — and now they are the ones sitting in the corner of the room telling their own child that it is going to be fine, and meaning it, because they know.
We have watched Carmel Valley go from new construction to established community, and we have watched the same thing happen to a whole generation of our patients. It is the single best thing about staying in one place.
What happens when kids stop being kids
Most pediatric practices hand you a referral and a handshake somewhere around age 18. We never liked that.
The transition out of pediatric care is exactly the wrong moment to hand a young adult a phone number and hope for the best. It is the age when people fall out of the habit entirely — they leave for school, they lose the appointment a parent used to book for them, and they resurface at 27 with five years of catching up to do.
So we brought adult care in-house. Dr. Anna Grindberg, our general dentist, sees patients as they move out of adolescence and into adulthood — and keeps seeing them. Same office. Same front desk. Same records, uninterrupted. A high school senior can have their last pediatric visit and their first adult visit in the same building, with a team that has their entire history in front of them.
What that means in practice: we now have families where the grandparent drives, the parent gets their own cleaning, and the kid gets their checkup — three generations, one afternoon, one office. It has become one of our favorite things about the schedule.
Why we stay
Thirty years in one neighborhood is a choice. We have had plenty of chances to become something else — but staying put is the whole point.
Staying put is how you know that a family moved from Torrey Hills up to Pacific Highlands Ranch when their second kid was born. It is how you know a teenager's orthodontic history without opening a chart. It is how you show up at the school fundraiser and recognize half the room.
