At Coastal Kids, our board-certified pediatric dentists perform extractions gently and efficiently, with behavior guidance techniques calibrated to your child's age and anxiety level. We also plan proactively for what comes next — whether that's a space maintainer, orthodontic monitoring, or simply a conversation about the tooth fairy.
Evaluation & X-Ray
We take a targeted X-ray to see root length, proximity to permanent teeth, and any complicating anatomy before deciding on technique. We explain the full picture to you before scheduling treatment.
Numbing
We apply topical anesthetic first, then administer a local injection slowly. For anxious children, we offer nitrous oxide before numbing to reduce anticipatory anxiety. Your child will feel pressure during the extraction — not pain.
Extraction & Aftercare
Most baby tooth extractions are straightforward and take only a few minutes. We place gauze for biting pressure, go over aftercare instructions in detail, and talk to you about next steps — including whether a space maintainer is indicated.

When does a baby tooth need to be extracted?
The most common reasons are: severe decay that has spread to the nerve and can't be saved with a pulpotomy, a baby tooth that's blocking or delaying a permanent tooth from erupting, an abscessed tooth causing pain or swelling, or a tooth that hasn't fallen out naturally when it should have. We recommend extractions only when they're clearly the right clinical choice, not as a default.
Will my child need a space maintainer after an extraction?
It depends on the tooth and how close the permanent successor is to erupting. If a baby molar is extracted more than a year before its permanent replacement is expected, we often recommend a space maintainer to prevent neighboring teeth from drifting into the space. We discuss this at the same appointment and factor it into your treatment plan.
How long is recovery after a tooth extraction for kids?
Most children recover quickly — mild soreness for one to two days, manageable with children's ibuprofen or acetaminophen. We recommend soft foods for the first 24 hours and ask children to avoid straws (suction disrupts healing) and vigorous rinsing. The extraction site typically heals over in one to two weeks. We provide written aftercare instructions to take home.
Will my child be awake during the extraction?
Yes, for most straightforward extractions. We use local anesthesia to ensure they feel no pain, and nitrous oxide is available for anxious children. For complex extractions or children with significant dental anxiety or special healthcare needs, we may discuss sedation options including oral conscious sedation or referral to a hospital-based pediatric dental specialist.
